We use Survey Monkey as a host for our 360 Feedback reports and all evaluation surveys.  By taking part in or been a recipient of either a 360 Feedback survey or evaluation survey you acknowledge that the information you provide will be transferred to Survey Monkey for processing in accordance with their Privacy Policy and Terms.  Any personal information held in our Survey Monkey account is used only for the sole purpose it was collected for.
